DNA samples for minis hair

WE HAVE A FEW MINIS WE PULL THEIR HAIR SAMPLES FOR DNA TESTING, BUT I HAVE NO FORMS TO FILL OUT. I LOOK ONLINE WITH AMHA ALL I SAW WAS BLOOD SAMPLES FOR DNA. HOW MUCH IS IT FOR THE DNA HAIR SAMPLES, I AM THINKING IT WAS $40 BACK A FEW YEARS AGO.IF I CAN'T GET A FORM WHAT ALL DO I NEED TO PULL ON THE PAPERWORK TO SEND THE HAIRS IN WITH?

This is the link that brings up the form. It is an Adobe file. You send this form in with the work order and the fee and they will send you the "kit" to put the hair in and send back.

Yes, minimule is correct. It costs $41.00, and you eventually receive...... after a few months......the DNA report.
